Managing obesity in hypothyroid patients poses unique challenges. This study addresses the gap in literature by comparing GLP1 analogues and bariatric surgery within this demographic, utilizing data from the TriNetX network. In this comparative analysis, we evaluated 25,236 patients receiving GLP1 analogues (Cohort A) and 12,824 undergoing bariatric surgery (Cohort B). Key outcomes included mortality, hospitalization rates, incidence of myxedema coma, and thyroid function tests, analyzed using robust statistical methods. Survival rates were equally high in both cohorts. Cohort A exhibited a significantly lower risk of hospitalization compared to Cohort B. No significant differences were observed in the incidence of myxedema coma or in thyroid function test results. The lower hospitalization risk associated with GLP1 analogues suggests their potential advantage in the management of obesity in hypothyroid patients. These findings highlight the need for personalized treatment strategies and could influence future clinical guidelines for this patient group. Ongoing research is essential to further understand long-term outcomes and patient-specific responses to these treatments.
