Adrenocortical carcinoma (ACC) is a rare malignancy with a high morbidity and mortality. More than half of cases are functional and have some type of hormone overproduction with a very broad variety of symptoms. Thrombosis has been reported in a few cases and is associated with a grim prognosis. Here we present a case of a patient with newly diagnosed ACC presented with acute pulmonary embolism (PE) and recurrent deep vein thrombosis (DVT) in the setting of hypercortisolism.
