Background: Hamstring injuries are common in sports that involve sprinting with sudden stops and starts. The hamstring muscles extend along the thigh, spanning both the hip and knee joints. Methods: Case report. Results: We present a case in which we used ultrasound guidance to perform a hydrodissection procedure on the distal hamstring. A 29-year-old male athlete sought medical attention due to a distal hamstring injury that had not only forced him to cease sports activities but had also negatively affected his work performance. Despite receiving physiotherapy, his condition was improving at a slow pace. After pinpointing the specific location of the injured distal hamstring muscles, we employed an ultrasound-guided hydrodissection technique using a solution consisting of 0.125% chirocaine and 40 mg of triamcinolone. Notably, the injection was targeted solely within the fascial planes. The patient reported a sustained improvement with early return to full sporting participation. Conclusions: The study's objective is to facilitate the quick alleviation of symptoms in athletes and enable their prompt return to sports. This method has the potential to assist in the management of hamstring injuries.
