Introduction Multiple myeloma is the second most common haematological malignant tumour. While the general disease is referred to as multiple myeloma, the solitary manifestation is defined as plasmacytoma [1]. Occurrence in the upper respiratory system is rare [2], but then has a good prognosis [3]. A 59-year-old patient presented with long-standing nasal obstruction on the right side. Examinations revealed that the right nasal cavity was almost completely obstructed by a dark, polypoid mass. Intraoperatively, the tumour tissue originated from the mucosa of region IV of the medial septum on the right side. The underlying bone of the lamina papyracea appeared intact. Due to the previously unknown histology, complete resection of the tumour masses was performed without bone resection.
