Increased age at the time of ileal orthotopic neobladder may also have a negative impact on HRQOL. We aimed to determine a data-based optimal age threshold at the time of RC with ileal orthotopic neobladder, after which HRQOL worsens. METHODS: We retrospectively analyzed all BCa patients that fulfilled the EORTC QLQ-C30 questionnaire preoperatively and at 3months, as well as 12 months after RC with ileal orthotopic neobladder in our department between 2013-2021. General health status (GHS-score) was calculated according to EORTC QLQ-C30 guidelines, Patients receiving adjuvant chemotherapy or palliative RC were excluded. RESULTS: We included a total of 120 patients (mean age 66±9.6) undergoing RC with ileal orthotopic neobladder. Their GHS preoperatively was 64±23, at three months postoperatively 64±20, and at twelve months 68±23. Overall, 80 (67%) patients presented an increase of the GHS at twelve months compared to their preoperative values. Interestingly, the International Consultation on Incontinence Questionnaire did not present statistically significant differences between patients with increase of the GHS at twelve months versus those with decrease of the GHS at twelve months (2.5±3.2 versus 2.8±3.5, p=0.57, respectively). Using ROC analysis with the Youden's index, we defined a threshold of 70 years, after which RC with ileal orthotopic neobladder leads to worse GHS at twelve months postoperatively. CONCLUSIONS: To our knowledge this study is the first to define an exact threshold based on HRQOL, after which RC with ileal orthotopic neobladder should be avoided and proper counseling regarding a potential decrease in quality of life should be performed.
