Although previous studies suggest a role for cytokines, the specific mechanisms by which elevated cytokine levels influence fibrogenesis is still unclear. Our objective is to evaluate inflammatory urinary cytokine levels in USD patients to discern a profile specific to USD patients and explore if these can be used as potential biomarkers to monitor disease progression as well as recurrence. METHODS: First catch urine samples were collected from USD patients (n=15) and 10 healthy controls (n=10), and 25uL samples were aliquoted for multiplex cytokine analysis. A Meso Sector Diagnostics (MSD) customizable U-Plex assay kit (CAT#: K15067M-1) was used to evaluate inflammatory cytokines (IFN-β, IFN-γ, IL-10, IL-1β, IL-2, IL-6, IL-8, MCP-1, TNF-α, TNF-β). Samples were simultaneously assayed for these 10 cytokines of interest with the MESO SECTOR S 600MM (MSD Maryland, USA). RESULTS: Our results showed that 5 out of 10 inflammation-related urinary cytokines (IL1-β, IL-6, IL-8, IL-10, TNF-β) were significantly elevated in USD patients (Figure 1; p<0.05). IL-8 was particularly high (over 150 folds when compared to controls). Increased IL-6 levels correlated with strictures diagnosed over 5 years ago (p=0.03). IL-8, IL-10, and TNF-β showed a significant correlation with history of prior transurethral interventions. Patients who underwent multiple procedures had significantly lower IL-10 and TNF-β, while patients who didn't receive any intervention had significantly lower values of IL-8. CONCLUSIONS: This is the first multiplex urinary cytokine analysis of USD patients, and our results indicate a relationship between each significant inflammatory urinary cytokine and urethral inflammation. Our findings suggest that urinary cytokine profile may serve as a non-invasive, sensitive, and specific biomarker that will enable diagnosis of USD occurrence and recurrence.
