To investigate the local oncologic and survival outcomes of cryoablation ablation (CA) of T1b renal tumors. We retrospectively reviewed our renal ablation data base and included only patients with renal tumors larger than 4 cm who underwent CA from January 2008 through December 2021. We collected patients and tumor characteristics, procedure details, pathology. Survival rates were estimated using the Kaplan-Meier method. Of the 40 patients who met our inclusion criteria (24 male and 16 female, median age 72 years), 16 patients (40%) has another non renal primary malignancy. Forty one procedures were performed for 41 renal tumors of a median tumor size of 4.7 cm. Most tumors (85%) were biopsy proven renal cell carcinoma. Fifteen patients underwent only CA while 26 patients underwent combined embolization and CA. Two patients (5%) developed recurrence at the ablation zone. One patient had repeat ablation and one decided to continue his care in another facility. The median OS was 5.1 years. The 5-year local-recurrence- and disease-free survival were 92% and 86%, respectively. CA is an effective treatment option for the management of T1b renal tumors in non-surgical candidates. Long-term follow up revealed excellent local oncologic control and survival outcomes.
