Case report highlights closure of cyclodialysis cleft in a glaucoma patient using intraocular lens, indicating potential effectiveness of this approach.
We present the case of a 54year-old male with openangle glaucoma who exhibited decreased vision, a shallow anterior chamber, and relatively low intraocular pressure (IOP, 9 mmHg) after undergoing Kahook dual-blade (KDB) goniotomy. CDC was confirmed and monitored using ultrasound biomicroscopy (UBM). Bscan ultrasonography revealed a choroidal detachment. The patient underwent phacoemulsification with in-thebag intraocular lens(IOL) implantation, with the haptics specifically oriented toward the area of the CDC. Two weeks postoperatively, the patient’s vision improved, IOP increased, and the anterior chamber deepened. Both UBM and gonioscopy confirmed the closure of the CDC, and B-scan ultrasonography revealed the resolution of choroidal detachment. Our report demonstrates that for KDBinduced CDC in patients with concomitant cataracts, phacoemulsification combined with in-thebag IOL implantation may represent one potential treatment approach, particularly when other conservative measures fail.
