ABSTRACT An impenetrable proximal cap complicates chronic total occlusion (CTO) percutaneous coronary intervention (PCI) and is often an indication for the retrograde approach. When also the distal cap of the CTO is impenetrable, very few options remain to recanalize the vessel.
