Abstract Background Endoscopic treatment of obesity still represents a challenge in terms of training and outcomes. A few years from now, endoscopic bariatric techniques could become more attractive than surgical ones, especially in the context of the emergence of new drugs. The scalability and environment issues, as well as the detrimental effects of surgery have also created a window of opportunity for procedures like the ESG (Endoscopic Sleeve Gastroplasty). Methods The Endoply device is a new tool for Endoplication, currently under clinical trial (pilot study in 10 patients), and posesses ergonomic features that places it in a position for a rapid access to interesting opportunities: 1) Assisting the procedure with a digital twin, 2) Carrying on with an externally placed optical device instead of a regular endoscope. Experimental proof of concept will be presented.
