Abstract

症例は70歳男性．64歳時に十二指腸球部癌に対して胃幽門側および十二指腸球部切除術を行った．病理診断結果は乳頭腺癌であり，郭清リンパ節に２個の転移を認めた．その後は定期的に経過観察していたが，十二指腸癌術後６年３カ月時に血清CA19-9値が114.1 U/mlと上昇し，CT検査で膵頭部に石灰化を伴う乏血性腫瘍を認めた．PET/CT検査でも同部位に著明なFDGの集積を認め，十二指腸癌の術前には血清CA19-9値の上昇はなく，術後６年以上経過していることなどから膵頭部癌を疑い膵頭十二指腸切除術を行った．組織学的検索では膵腫瘍の組織型は中分化型腺癌で十二指腸癌の組織型と異なっており，十二指腸断端にも腫瘍の露出はなく，局所再発やリンパ節再発を疑う所見も認めなかったため，膵原発の浸潤性膵管癌と診断した．十二指腸球部癌と膵頭部癌との異時性重複癌は極めて稀な症例であり，文献的考察も加えて報告する．
