Pulsed field ablation triggers acute ventricular fibrillation during posterior wall isolation in case report.
Case Report (n=1)
Pulsed field ablation for persistent atrial fibrillation can rarely induce immediate, life-threatening ventricular fibrillation during posterior wall ablation, necessitating continuous electrocardiographic monitoring and readiness for immediate defibrillation.
Pulsed field ablation (PFA) has rapidly gained adoption as a novel energy modality for catheter ablation of atrial fibrillation (AF), owing to its myocardial selectivity and favorable safety profile compared with conventional thermal ablation techniques (1). By inducing irreversible electroporation, PFA enables effective pulmonary vein isolation (PVI) with minimal collateral injury to adjacent structures, and its use has expanded to more extensive ablation strategies including posterior wall isolation (PWI), particularly in patients with persistent AF (2-3).

Iimura et al. (2026) conducted a case report in Persistent Atrial Fibrillation (n=1). Pulsed field ablation (PFA) was evaluated on Occurrence of ventricular fibrillation. Pulsed field ablation induced immediate ventricular fibrillation during posterior wall isolation in a patient with persistent atrial fibrillation, highlighting a rare proarrhythmic risk.
