We report a case of a patient with treated bilateral non-proliferative diabetic retinopathy (NPDR) with diabetic macular edema (DME) who developed full thickness macular hole (FTMH) in the right eye which resolved spontaneously. A 45-year-old lady presented with diminution of vision in both eyes. She was diagnosed to have bilateral moderate NPDR with DME and was treated with multiple intravitreal anti-vascular endothelial growth factor (VEGF) injections. Of interest was the development of FTMH in right eye which resolved spontaneously without surgical intervention. All the while, patient maintained optimum blood sugar level control. Spontaneous resolution of FTMH is a rare outcome in patients with DME.
