Abstract A 48-year-old woman, 14 years after orthotopic heart transplantation, presented with recurrent syncope and complete atrioventricular block. During pacemaker implantation, intracardiac electrograms showed only atrial fibrillation despite organized atrial activity on surface electrocardiography. Electroanatomical mapping revealed complete donor–recipient atrial dissociation, with recipient atrial fibrillation and preserved donor sinus rhythm conducting to the atrioventricular node. This case highlights dual atrial rhythms after transplantation and the importance of mapping to guide atrial lead placement.
