This case series describes a combined approach using pedicled buccal fat pad transfer and implant-supported milled bar overdentures for the functional rehabilitation of older edentulous patients with oncological maxillary defects. All four patients underwent partial maxillectomy. The oroantral communication was closed using pedicled buccal fat pads, and the implants were placed simultaneously to facilitate subsequent occlusal restoration. The wounds were covered with an artificial dermis. In all cases, implant-supported milled bar overdentures were successfully delivered within one year. No oroantral fistulas were observed and all patients were able to tolerate a standard diet. One patient developed mild peri-implant mucositis, but there was no evidence of significant bone resorption. The milled bar attachments demonstrated stable retention and reduced prosthesis instability, even in cases with complex anatomical conditions. This treatment strategy offers a minimally invasive and effective approach that combines surgical reconstruction with prosthetic rehabilitation in older edentulous patients with maxillary cancer.
