A 52-year-old female presented with an incidental discovery of a vaginal vault mass during routine imaging for thyroid disease follow-up. She was previously treated for papillary thyroid cancer (PTC) with metastases to the mediastinum, managed through total thyroidectomy and radioactive iodine-131 therapy. Histopathological evaluation of the vaginal vault mass revealed recurrence of PTC, confirmed by thyroglobulin immunostaining positivity. The absence of the BRAF V600E mutation was noted. This case highlights an extremely rare site of PTC recurrence, emphasizing the need for vigilance in follow-up and the potential for atypical metastatic patterns. We present the diagnostic challenges and management considerations for this unusual presentation.
