Abstract

【症例】81歳女性．72歳頃より両膝痛を自覚し，徐々に増悪，術前1年程からは独歩困難となり家の中ではバケツを両手で持ち杖代わりに移動するようになった．右は内反，左は外反，いずれもKL分類grade 4の変形を認めた．入院時は高度の可動域制限があり，つたい歩きも困難な状態のため両側人工膝関節置換術（TKA）を施行する方針となった．疼痛が強く可動域制限が著しい側は内反右膝であったが，Cross-legが起こるのを危惧し，外反左膝から手術を行った．2週後，内反右膝のTKA施行．術後2ヶ月でピックアップ歩行器使用にて退院．術後1年での可動域は良好，独歩可能である．【考察】歩行不能な高度な変形のwindswept deformityに対して両側TKAを行ったところ経過良好であった．今回の症例ではCross-legを危惧し外反側から手術を行ったが，術後内反側の疼痛が強くリハビリに難渋した．手術の順番としては疼痛の強い膝からの手術が望ましいかと考えられた．
