Ischemic comorbidity cluster linked to ~25% higher risk of ICU admission in acute heart failure.
Why the study?
Acute heart failure frequently coexists with multimorbidity, and comorbidity clusters associate with distinct clinical outcomes, but data from the Americas remain limited.
Cohort (n=1,638)
Yes
Distinct multimorbidity clusters in patients with acute heart failure exhibit varying clinical profiles and are independently associated with differing risks of ICU admission, highlighting the importance of comorbidity-based risk stratification.
May support comorbidity-based risk stratification in acute HF; leaves open whether clusters should guide targeted interventions.
BACKGROUND: Acute heart failure (HF) frequently coexists with multimorbidity. Comorbidity clusters have been associated with distinct clinical outcomes, but data from the Americas remain limited. We aimed to identify comorbidity clusters in patients with acute HF and assess their association with in-hospital outcomes. METHODS: This cohort study applied latent class analysis to patients from the AMERICCAASS Registry (Registro Americano de Insuficiencia Cardiaca Ambulatoria o Agudamente descompensada). Adjusted Poisson regression assessed in-hospital mortality and intensive care unit (ICU) admission, and multivariable analysis identified independent predictors of ICU admission. Severe valvular insufficiency, peripheral artery disease, dyslipidemia, cancer, acute coronary syndrome, anemia, and cardiogenic shock were associated with higher ICU admission risk. CONCLUSIONS: Patients with acute HF in the Americas exhibit distinct comorbidity patterns associated with different clinical profiles and ICU admission rates, which may inform risk stratification and management strategies. Arango‐Ibanez et al. (2026) conducted a cohort in Acute heart failure (n=1,638). Ischemic comorbidity cluster (Cluster 3) vs. Young comorbidity cluster (Cluster 1) was evaluated on Intensive care unit (ICU) admission (aRR 1.25, 95% CI 1.12-1.39, p=<0.001). In patients hospitalized with acute heart failure, the Ischemic comorbidity cluster was associated with a higher risk of intensive care unit admission compared to the Young cluster (aRR 1.25).
