Abstract Background: Chronic liver disease (CLD) is a major global health burden, characterized by progressive deterioration of liver function over time. Aim: The aim of this study was to assess the quality of life (QOL), fatigue, resilience, anxiety, and depression in CLD patients. Materials and Methods: This cross-sectional study was conducted on patients with CLD selected by purposive sampling. Sociodemographic details were recorded. The Alcohol Use Disorders Identification Test-Consumption (AUDIT-C), Chronic Liver Disease Questionnaire, Hospital Anxiety and Depression Scale (HADS), Connor-Davidson Resilience Scale-2, and Fatigue Severity Scale were applied to the patients. Results: The study examined 175 patients (100 males and 75 females). Abnormal levels of anxiety or depression were seen in 37.1%. Moderate and severe levels of fatigue were reported by 40% and 37.1% of the patients, respectively. A moderate QOL was reported by 48.6% of patients, while 34.3% experienced poor QOL. Based on AUDIT-C, 51.4% of patients were classified as alcohol users and 48.6% as nonusers. Child–Pugh Score (CTP) showed a strong negative correlation with QOL. Significant positive correlations were found between CTP and anxiety and depression scores. Multiple regression showed that anxiety, depression, longer alcohol use, and poor QOL were significant predictors of higher Child–Pugh scores; depression and age predicted fatigue, while resilience was associated with lower fatigue and alcohol use severity. Conclusion: Patients with CLD experience heightened levels of anxiety, depression, fatigue, and a noticeable decline in their QOL. Female patients exhibited higher levels of anxiety, depression, and fatigue compared to males.
