Traumatic endophthalmitis following open-globe injury (OGI) with retained intraocular foreign body (IOFB) carries a high risk of vision loss. We describe the first case of post-traumatic endophthalmitis caused by Paenibacillus durus in a 24-year-old mechanic. Following metal trauma, orbital CT revealed a mid-vitreal IOFB; the patient underwent corneal repair, lensectomy, vitrectomy, and intravitreal vancomycin and ceftazidime. Automated BACTEC FX40 flagged positive; upon subculture on blood agar, it yielded turmeric yellow colonies, identified as P. durus by VITEK 2 Compact, with vancomycin sensitivity. Over four weeks, inflammation resolved, and the patient achieved 20/20 aphakic vision. This case expands the spectrum of ocular pathogens and underscores the value of rapid diagnosis and targeted therapy.
