Case reports reveal treatment challenges post glaucoma surgery in two patients, indicating the need for careful preoperative counseling.
We present two cases of cyclodialysis cleft (CDC) following minimally invasive glaucoma surgery, both unresponsive to conservative treatment. The first, a 50-year-old man with prior vitreoretinal surgery, developed CDC post gonioscopy-assisted transluminal trabeculotomy, treated successfully using the modified sewing machine technique. The second, a 20-year-old with juvenile open-angle glaucoma, developed CDC post Kahook-dual-blade goniotomy and underwent endocyclotamponade with Cionni ring. Despite anatomical success, both eyes experienced significant IOP spikes postrepair, needing further surgical intervention. This highlights the importance of preoperative counseling as prolonged hypotony in glaucomatous eyes may impair trabecular function, requiring further intervention despite successful CDC closure.
