Case report and literature review reveals diagnostic challenges of posterior scleritis in vascular inflammation cases.
Posterior scleritis, particularly the nodular variant, is often a diagnostic challenge due to misleading clinical presentations. Multimodal imaging and a high index of suspicion are required to establish a diagnosis. We report a case of nodular posterior scleritis in a young male that initially appeared like a choroidal mass. Thorough workup led to a diagnosis of granulomatosis with polyangiitis. The clinical presentation, dilemmas, and approach to diagnosis are discussed along with review of literature of similar cases of posterior scleritis.
