A 40-year-old male with neurocysticercosis, on pre-albendazole screening, showed a subretinal cyst in the right eye [Fig. 1a]. He underwent pars plana vitrectomy with cyst removal [Fig. 1b], following which he was started on Injection dexamethasone 8 mg 3times/day, tablet albendazole 400 mg 2times/day for systemic involvement [Fig. 1c and d]. It is crucial to manage intraocular cysticercosis before initiating albendazole therapy to prevent severe posterior uveitis resulting from cyst death.[1] This clinical image highlights the need for high clinical suspicion, thorough history-taking, and targeted diagnostics to identify rare conditions. The image illustrates varied and rare ocular presentations of cysticercosis.Figure 1: (a) Fundus photo of right eye shows subretinal cyst with scolex (Arrow), (b) MRI – Axial plane (T2W image) shows cyst in the extraocular muscle belly, (c) Fundus photo of right eye Chorioretinal scar post-surgery (Arrow), (d) Magnetic Resonance Imaging (MRI)– Axial plane (T2W image) shows multiple cysts in the brain (Arrow)Authors’ contributions Dr. Pedarla.
