Case report reveals serious vision impairment due to congenital retinal macrovessel with complications.
Congenital retinal macrovessel (CRM) is a rare vascular anomaly usually asymptomatic. We report a 14-year-old male presenting with severe unilateral vision loss caused by a superotemporal congenital retinal macrovessel associated with an epiretinal membrane, loss of the foveal pit, and reduction of the foveal avascular zone. Multimodal imaging, including fundus photography, fluorescein angiography, optical coherence tomography (OCT), and OCT angiography, confirmed the structural disruption. This case highlights that CRM, though often benign, may lead to significant vision impairment when complicated by macular abnormalities. Individualized monitoring and consideration of surgical intervention may be warranted.
