Combined echo S/D ratio and venous pressure linked to ~65% higher odds of Fontan diastolic dysfunction.
Why the study?
The diagnostic value of noninvasive parameters for diagnosing diastolic dysfunction and impaired pulmonary reserve in patients with Fontan circulation remains unclear compared with gold-standard exercise cardiac catheterization.
Does noninvasive evaluation using echocardiography and peripheral venous pressure predict invasive diastolic dysfunction and pulmonary vascular reserve in adults with Fontan circulation?

A novel, noninvasive algorithmic approach using echocardiographic systole-to-diastole ratio and peripheral venous pressure correlates strongly with invasive filling pressures, enabling feasible screening for diastolic dysfunction and pulmonary vascular reserve in Fontan patients.
May support noninvasive diastolic assessment in Fontan patients; leaves open prospective validation before clinical adoption.
BACKGROUND: Exercise cardiac catheterization is the gold standard for diagnosing diastolic dysfunction and impaired pulmonary reserve in patients with Fontan circulation. The diagnostic value of noninvasive parameters is unclear. METHODS: Patients who underwent simultaneous exercise hemodynamic evaluation and peripheral venous pressure (PVP) measurement were included. Echocardiographic surrogates for ventricular filling pressure were compared with invasive measures. Diastolic dysfunction was defined invasively as either resting pulmonary artery wedge pressure ≥12 mm Hg or exercise pulmonary artery wedge pressure >20 mm Hg. RESULTS: =0.01). CONCLUSIONS: This study presents a simple, noninvasive approach for evaluating diastolic function and pulmonary vascular reserve in patients with a Fontan circulation.

Cao et al. (2025) conducted a cohort in Fontan circulation (n=29). Echocardiographic S/D ratio and peripheral venous pressure (PVP) was evaluated on Diastolic dysfunction defined by rest and exercise PAWP (OR 1.65, 95% CI 1.15-2.74, p=0.02). A combination of echocardiographic S/D ratio and peripheral venous pressure accurately predicted diastolic dysfunction in adults with Fontan circulation, yielding a sensitivity of 78% and specificity of 95%.
