Overview

Prospective cohort study observes that higher physical activity and lower sedentary time preserve intrinsic capacity in older adults, indicating key behavioral targets for healthy aging.

Key Points

  • To evaluate the prospective associations of device-measured physical activity and sedentary behaviour with longitudinal changes in intrinsic capacity among community-dwelling older adults.
  • Prospective population-based cohort study (Seniors-ENRICA-2) assessing 2477 community-dwelling adults aged 65–94 years in Spain recruited between Dec 2, 2015, and Nov 23, 2017.
  • Movement behaviors (sedentary time, light physical activity, and moderate-to-vigorous physical activity [MVPA]) were measured via baseline accelerometry.
  • Intrinsic capacity composite scores across six domains (vitality, cognition, psychological, locomotion, vision, and hearing) were assessed at baseline and followed up over a median of 2.3 years (n=1463) and 5.5 years (n=940).
  • Each 15-minute increase in baseline MVPA was associated with an increase in intrinsic capacity (mean percentage change [MPC] 0.63%, 95% CI 0.06 to 1.21), whereas sedentary behaviour was associated with a decline (MPC per 15 min -0.29%, 95% CI -0.57 to -0.01); light physical activity showed no significant association (MPC -0.39%, 95% CI -0.85 to 0.07).
  • Compared with the lowest tertile, the highest and intermediate tertiles of MVPA were associated with higher intrinsic capacity (MPC 4.83%, 95% CI 1.98 to 7.75, and 5.44%, 95% CI 2.52 to 8.45, respectively).
  • Compared with the highest tertile of sedentary time, the lowest (MPC 5.48%, 95% CI 2.88 to 8.02) and intermediate (MPC 5.73%, 95% CI 3.16 to 8.22) tertiles showed significant improvements in intrinsic capacity.
