High MR recurrence after UMRA plus CABG cautions against routine use; leaves open whether better selection or techniques improve durability.

Combined undersizing mitral ring annuloplasty and CABG for chronic ischemic mitral regurgitation is associated with significant recurrence of mitral regurgitation and adverse ventricular remodeling at 3 to 5 years.
Gelsomino et al. (2007) conducted a cohort in Chronic ischaemic mitral regurgitation (CIMR) (n=251). Combined undersizing mitral ring annuloplasty (UMRA) and coronary artery bypass grafting (CABG) was evaluated on Echocardiographic mitral regurgitation (MR) recurrence at 5 years (p=<0.001). Combined undersized mitral ring annuloplasty and CABG for chronic ischemic mitral regurgitation resulted in MR that was stable at 1 year but significantly re-increased at 3 and 5 years (P<0.001).
