Key result
Peak left atrial longitudinal strain <33.4% predicts incident atrial fibrillation.
Why the study?
AF is the commonest supraventricular arrhythmia in adults, and timely diagnosis seems to ameliorate prognosis, motivating investigation into the association between peak left atrial longitudinal strain and new onset AF in the general population.
Does peak left atrial longitudinal strain (PALS) predict incident atrial fibrillation in the general population?
Meta-Analysis
Does peak left atrial longitudinal strain (PALS) predict incident atrial fibrillation in the general population?
Effect estimate: Sensitivity 64%, Specificity 69% (95% CI 46-79 for sensitivity, 63-75 for specificity)
Lower peak left atrial longitudinal strain (<33.4%) has moderate diagnostic accuracy for predicting incident atrial fibrillation in the general population, supporting an opportunistic screening approach.
BACKGROUND: Atrial fibrillation (AF) is the commonest supraventricular arrhythmia in adults. Timely AF diagnosis seems to ameliorate patients prognosis. PURPOSE: To investigate the association between peak left atrial longitudinal strain (PALS) and new onset AF in the general population. OBJECTIVES: We searched major electronic databases for articles assessing the relationship between PALS and incident AF. RESULTS: : 83%). According to the diagnostic accuracy meta-analysis, PALS <33.4% presents 64% (95%CI: 46-79%) sensitivity and 69% (95%CI: 63-75%) specificity. CONCLUSIONS: In a relatively healthy population, lower levels of PALS were significantly associated with incident AF. The overall diagnostic accuracy was moderate. Lower levels of PALS seem to justify an opportunistic - rather than a systematic-screening approach. These findings could allow more efficient utilisation of healthcare resources.

Anagnostopoulos et al. (2024) conducted a meta-analysis in Atrial fibrillation. Peak left atrial longitudinal strain (PALS) was evaluated on Incident atrial fibrillation (Sensitivity 64%, Specificity 69%, 95% CI 46-79 for sensitivity, 63-75 for specificity). Peak left atrial longitudinal strain <33.4% was associated with incident atrial fibrillation, demonstrating 64% sensitivity (95% CI 46-79%) and 69% specificity (95% CI 63-75%).
