To evaluate whether screening sigmoidoscopy is associated with a reduction in mortality from colorectal cancer.
Conducted a retrospective case-control study reviewing medical records of members within the Greater Marshfield Community Health Plan (GMCHP).
Identified 66 case subjects who died of large-bowel cancer between 1979 and 1988, comparing them to 196 randomly chosen control subjects matched for age, gender, and enrollment duration.
Screening sigmoidoscopy was documented in 10% of cases compared to 30% of controls, with a single examination associated with decreased colorectal cancer death (odds ratio [OR] = 0.21; 95% CI, 0.08–0.52).
Risk reduction was confined to malignancies of the rectum and distal colon, whereas fecal occult blood testing and digital rectal examinations showed no significant association with mortality reduction.
