Key result
Peak frequency analysis characterizes AF substrate and identifies overestimated LVZs with 0.76 AUC.
Why the study?
Characterizing the atrial fibrillation substrate can guide ablation strategies, leading to the evaluation of a novel parameter, peak frequency, for substrate characterization.
Does peak frequency analysis improve the characterization of atrial fibrillation substrate?
Observational
Does peak frequency analysis improve the characterization of atrial fibrillation substrate?
Effect estimate: AUC 0.76
p-value: p=<.001
Peak frequency analysis of electrograms can effectively differentiate between fixed and functional remodeling, potentially reducing the overestimation of low voltage zones during AF mapping.
Background: Characterizing atrial fibrillation (AF) substrate can guide ablation strategies. Objective: A novel parameter, peak frequency (PF), was evaluated in its ability to characterize the substrate in AF. Methods: was defined as the highest frequency detected in the electrogram. Results: <.001) with an AUC of 0.76. A majority of LVZs identified only in AF OV maps exhibited PF compatible with that seen in non-LVZs, suggesting that PF analysis can pinpoint potential overestimations of LVZs. Conclusion: PF can effectively discern between sites of fixed remodeling, functional remodeling, and potential overestimations of LVZs. PF may thereby aid in better characterization of the substrate in AF.

Al‐Aidarous et al. (2025) conducted an observational in Atrial fibrillation. Peak frequency (PF) analysis was evaluated on Characterization of atrial fibrillation substrate and low voltage zones (LVZs) (AUC 0.76, p=<.001). Peak frequency analysis effectively characterized the atrial fibrillation substrate and identified potential overestimations of low voltage zones with an AUC of 0.76 (P<.001).
