Key result
Impaired 3D-GLS one year post-STEMI predicts ~53% greater long-term MACE risk.
Why the study?
While 2D and 3D speckle tracking echocardiography can predict prognosis after STEMI, the clinical significance of serial 3D-STE in predicting prognosis required investigation.
Does the deterioration of 3D-GLS measured by STE at 1 year after the onset of STEMI predict long-term prognosis?
Observational (n=272)
No
Does the deterioration of 3D-GLS measured by STE at 1 year after the onset of STEMI predict long-term prognosis?
Effect estimate: HR 1.53 (95% CI 1.35-1.74)
p-value: p=<0.0001
Deterioration of 3D-GLS measured by STE at 1 year post-STEMI is a strong predictor of long-term prognosis.
May refine post-STEMI risk stratification; hypothesis-generating and requires prospective validation before clinical use.
BACKGROUND: Two-dimensional (2D) and three-dimensional (3D) speckle tracking echocardiography (STE) after ST-elevation acute myocardial infarction (STEMI) can predict the prognosis. This study investigated the clinical significance of a serial 3D-STE can predict the prognosis after onset of STEMI. METHODS AND RESULTS: =36.7, P<0.0001). CONCLUSIONS: The deterioration of 3D-GLS measured by STE at 1 year after the onset of STEMI is the strongest predictor of long-term prognosis.

Iwahashi et al. (2021) conducted an observational in ST-elevation acute myocardial infarction (n=272). 3D-global longitudinal strain (3D-GLS) assessment vs. Conventional echocardiography and 2D-GLS was evaluated on Major adverse cardiac events (cardiac death and heart failure hospitalization) (HR 1.53, 95% CI 1.35-1.74, p=<0.0001). A 3D-global longitudinal strain >-13.1 at 1 year after ST-elevation myocardial infarction strongly predicted long-term major adverse cardiac events (HR 1.53).
