Implication

Supports PTCA feasibility in selected post-CABG patients; leaves open randomized comparison to reoperation.

Structured PICO

P
Population
84 consecutive patients with previous coronary artery bypass grafting (CABG) undergoing first PTCA of the saphenous vein bypass graft alone or with native vessels, mean age 60, 77% male, based in The Netherlands. Key inclusion: stable or unstable angina pectoris despite intensive pharmacological therapy, considered appropriate candidates for repeat bypass surgery.
I
Intervention
Percutaneous transluminal coronary angioplasty (PTCA) of saphenous vein bypass grafts (with or without native vessel PTCA)
O
Outcome
Death, non-fatal myocardial infarction, recurrent angina pectoris necessitating repeat bypass surgery or repeat angioplasty, and symptomatic improvement at median 2.1 years follow-upcomposite

PTCA of saphenous vein grafts is a feasible alternative to re-operation in selected post-CABG patients, though it is contraindicated in totally occluded grafts due to an unacceptably high risk of distal embolization and myocardial infarction.
