Key result
During and post-hemodialysis phases are linked to ~70% higher NSVT frequency versus between-dialysis periods.
Why the study?
Sudden cardiac death is more frequent after a long interdialytic interval and within 6 hours of hemodialysis, prompting investigation into whether arrhythmias relate to heart rate and variability changes across hemodialysis phases.
Are changes in heart rate and heart rate variability during different phases of hemodialysis associated with the occurrence of paroxysmal arrhythmias in patients with end-stage kidney disease?
Population
28 patients with end-stage kidney disease (mean age 54±13 years, 57% men)
Comparison
Different phases of hemodialysis and interdialytic intervals
Design
Prospective ancillary cohort study

May prompt closer rhythm monitoring around dialysis in ESKD; leaves open whether HR/HRV shifts are causal or actionable.
Cohort (n=28)
Are changes in heart rate and heart rate variability during different phases of hemodialysis associated with the occurrence of paroxysmal arrhythmias in patients with end-stage kidney disease?
Absolute Event Rate: 63% vs 37%
p-value: p=0.015
In patients with end-stage kidney disease, nonsustained ventricular tachycardia occurs more frequently during and immediately after hemodialysis, and is associated with sudden heart rate increases and parasympathetic withdrawal during the long interdialytic interval.
Rogovoy et al. (2019) conducted a cohort in End-stage kidney disease (n=28). Hemodialysis (during/posthemodialysis phase) vs. Pre-/between hemodialysis phase was evaluated on Nonsustained ventricular tachycardia (p=0.015). The during/post-hemodialysis phase was associated with a higher frequency of nonsustained ventricular tachycardia compared to the pre/between hemodialysis phase (63% vs 37%, P=0.015).
