Voltage-guided ablation in persistent AF patients >75 years of age yielded similar 36-month AF-free survival compared to younger patients, despite increased left atrial substrate remodeling.
Why the study?
Data on left atrial remodeling and low-voltage zone extent in elderly patients with atrial fibrillation undergoing ablation are scarce.
Does voltage-guided ablation provide similar AF-free survival in persistent AF patients >75 years of age compared to younger patients despite increased left atrial remodeling?

Does voltage-guided ablation provide similar AF-free survival in persistent AF patients >75 years of age compared to younger patients despite increased left atrial remodeling?
Voltage-guided ablation for persistent AF is effective and safe in patients >75 years of age, yielding similar 36-month AF-free survival compared to younger patients despite more extensive left atrial substrate remodeling.
May support voltage-guided ablation in elderly persistent AF patients; leaves open need for randomized confirmation.
Background: The prevalence of atrial fibrillation (AF) increases with age. The improvement in ablation techniques has widened the indications, particularly in elderly patients. Data on LA remodeling and low-voltage zone (LVZ) extent in this subgroup are scarce. Objective: We assessed the left atrial (LA) bipolar voltage, LVZ extent, and efficacy of voltage-guided ablation in a cohort of patients with persistent AF according to age. Methods: was defined as <0.5 mV. A propensity score-matching analysis was used to assess the impact of age on LA remodeling. After 36 months of follow-up, the AF-free survival rate after a single procedure was similar between the 2 groups. Conclusion: Patients >75 years of age with persistent AF display increased LA substrate remodeling than do younger patients. LA scar did not seem to negatively affect the results of substrate-guided ablation, and the complication rate was low.

Marzak et al. (2024) conducted a cohort in Persistent atrial fibrillation. Voltage-guided ablation vs. Younger patients was evaluated on AF-free survival rate after a single procedure. Voltage-guided ablation in persistent AF patients >75 years of age yielded similar 36-month AF-free survival compared to younger patients, despite increased left atrial substrate remodeling.
