Initial INR response accurately predicts 1-year warfarin maintenance dose.
Why the study?
The maintenance dose of warfarin varies widely among individuals, and although many predictive formulae exist, most are complex and not in practical use.
Does a simple formula based on initial response to low dosing accurately predict the maintenance dose of warfarin in outpatients?
Observational (n=127)

Does a simple formula based on initial response to low dosing accurately predict the maintenance dose of warfarin in outpatients?
The study aims to develop a simple, practical formula for predicting warfarin maintenance dose based on initial response to low dosing.
Answers are grounded in this paper and cite their sources.
May aid outpatient warfarin initiation; leaves open prospective validation before practice change.
Objective The pharmacodynamic effect of warfarin varies among individuals, and its maintenance dose is widely distributed. Although many formulae for predicting the maintenance dose of warfarin have been developed, most of them are complex and not in practical use.

Shoji et al. (2019) conducted an observational in Atrial fibrillation (n=127). Warfarin was evaluated on Correlation between predicted and actual maintenance dose of warfarin (exponential formula) (R2 0.795, 95% CI 0.726-0.851, p=<0.001). An exponential formula using the initial response of the INR to a 2 mg/day dose of warfarin accurately predicted the maintenance dose at 1 year (R2 0.795, p<0.001).
