Key result
Obesity is linked to up to ~100% higher heart failure risk versus normal weight across cohorts.
Why the study?
Heart failure and atrial fibrillation frequently coexist, but differences in the obesity-mediated association between HF and HF-related AF among Asians and Europeans had not been evaluated.
Is obesity associated with an increased risk of heart failure and related atrial fibrillation in Korean and UK adults?
Cohort (n=871,684)
Yes
Is obesity associated with an increased risk of heart failure and related atrial fibrillation in Korean and UK adults?
Effect estimate: HR 2.00 (95% CI 1.69-2.37)
p-value: p=<0.001
Obesity is associated with an increased risk of heart failure and related atrial fibrillation across both Asian and European populations.
Supports targeted obesity interventions for HF prevention; extends cross-ethnic evidence but remains hypothesis-generating.
PURPOSE: Heart failure (HF) and atrial fibrillation (AF) frequently coexist, with over 50% patients with HF having AF, while one-third of those with AF develop HF. Differences in obesity-mediated association between HF and HF-related AF among Asians and Europeans were evaluated. MATERIALS AND METHODS: Using the Korean National Health Insurance Service-Health Screening (K-NHIS-HealS) cohort and the UK Biobank, we included 394801 Korean and 476883 UK adults, respectively aged 40-70 years. The incidence and risk of HF were evaluated based on body mass index (BMI). RESULTS: =0.277). CONCLUSION: Obesity was associated with an increased risk of HF and HF-related AF in both Korean and UK populations. The higher incidence in the UK population was likely due to the higher proportion of obese individuals.

Lee et al. (2023) conducted a cohort in Heart failure and atrial fibrillation (n=871,684). Obesity (BMI ≥30 kg/m2) vs. Normal weight (BMI 18.5 to <25.0 kg/m2) was evaluated on Incidence of heart failure (UK Biobank cohort) (HR 2.00, 95% CI 1.69-2.37, p=<0.001). Obesity increased the risk of heart failure compared to normal weight in both Korean (HR 1.82) and UK (HR 2.00) cohorts, with higher overall incidence in the UK driven by greater obesity prevalence.
