Catheter ablation emerges as a first-line treatment over medication for paroxysmal AF.
Why the study?
Recent randomized trials of catheter ablation with improved equipment, techniques, and operator experience may challenge the historical practice of reserving ablation as second-line therapy after anti-arrhythmic medication failure.
Does catheter ablation improve outcomes compared to anti-arrhythmic medications as first-line therapy in patients with paroxysmal atrial fibrillation?
Does catheter ablation improve outcomes compared to anti-arrhythmic medications as first-line therapy in patients with paroxysmal atrial fibrillation?
This discussion highlights recent trial evidence supporting catheter ablation as a potential first-line therapy for paroxysmal atrial fibrillation, challenging the traditional approach of reserving it for patients who fail anti-arrhythmic medications.
May support earlier ablation consideration in paroxysmal AF; leaves open need for confirmatory long-term data.
The past decade has seen incremental improvements in the equipment and techniques used in catheter ablation for atrial fibrillation (AF). In parallel with this, the volume of procedures has expanded, producing operators and institutions with a far deeper experience. Improved technology in experienced hands has a powerful effect on the outcome of ablation. A set of recent randomized trials illustrates this effect and should revolutionize our approach to paroxysmal AF.1–4 Historically, the major randomized clinical trials on AF ablation selected patients who had failed anti-arrhythmic medications. The selection criteria were in line with international guidelines for AF ablation.5,6 Historically, ablation was reserved for 2nd line use, based on a perception that pharmacological treatment might be safer. Those trials established the efficacy of ablation for paroxysmal AF after failure of anti-arrhythmic medication. Current European guidelines therefore support AF ablation if one anti-arrhythmic medication fails to control the...

Leung et al. (2021) conducted a review in Paroxysmal atrial fibrillation. Catheter ablation vs. Anti-arrhythmic medications was evaluated. Recent randomized trials suggest that catheter ablation should be considered as a first-line treatment for paroxysmal atrial fibrillation rather than reserving it for after medication failure.
