【背景】拡大肺静脈隔離（extensive encircling pulmonary vein isolation：PVI）術中の麻酔管理は安全に手技を行う上で不可欠であるが，鎮静，鎮痛薬の使用に定まった方法はない．当院では，プロポフォールにペンタゾシンあるいはフェンタニルを併用している．今回，PVIにおけるフェンタニルの有効性をペンタゾシンと比較検討した．【対象と方法】2016年11月～2018年12月に当院でPVIを行った225人の心房細動患者を対象とした．全例に，鎮静薬としてプロポフォールを使用した．2016年11月～2017年5月にペンタゾシンを併用したペンタゾシン群（P群）62症例，2017年6月～2018年12月にフェンタニルを併用したフェンタニル群（F群）163症例の2群間で，患者背景，術中のプロポフォール投与量，追加投与回数，術後の覚醒時間に関して比較した．【結果】患者の平均年齢は66.9歳で，74％が男性であった．プロポフォールの総投与量［P群：17.5mg/kg（15.6～21.2mg/kg）vs F群：21.6mg/kg（18.4～25.1mg/kg），p＜0.01］，初期導入時急速投与量［P群：1.9mg/kg（1.6～2.2mg/kg）vs F群：3.4mg/kg（2.9～4.4mg/kg），p＜0.01］はF群が有意に多かった．左肺静脈隔離開始からPVI終了までのプロポフォール追加投与量［P群：0.8mg/kg（0.5～1.1mg/kg）vs F群：0.4mg/kg（0～0.8mg/kg），p＜0.01］，追加投与回数［P群：2.0回（1.0～3.0回）vs F群：1.0回（0～2.0回），p＜0.01］はF群が有意に少なかった．プロポフォール中止から抜管までに要した時間（覚醒時間）［P群：17.0分（12.3～21.0分）vs F群：17.0分（13.0～21.5分），p＝0.54］，プロポフォール中止から退室までに要した時間（退室時間）［P群：40.0分（32.3～47.0分）vs F群：40.0分（35.0～46.0分），p＝0.64］，手術時間［P群：189分（172～222分）vs F群：184分（162～209分），p＝0.11］は2群間で有意差はなかった．【結語】フェンタニルはペンタゾシンに比べ，術中のプロポフォール追加投与量，回数を減少させる可能性がある．
