Multiscale fluid dynamics model simulates severe CTEPH hemodynamics and identifies optimal targets for BPA.
A novel computational framework may help optimize lesion selection for balloon pulmonary angioplasty in patients with chronic thromboembolic pulmonary hypertension.
May inform lesion selection for BPA in CTEPH; leaves open whether model-guided procedures improve outcomes.
This article presents novel computational framework for predicting pulmonary hemodynamics in chronic thromboembolic pulmonary hypertension. The mathematical model is used to identify the optimal target lesions for balloon pulmonary angioplasty, combining simulated pulmonary artery pressure, wave intensity analysis, and a new quantitative metric of flow heterogeneity.

Colebank et al. (2021) studied Chronic thromboembolic pulmonary hypertension (CTEPH) (n=1). Simulated balloon pulmonary angioplasty (BPA) vs. Simulated normotensive and CTEPH disease scenarios was evaluated on Simulated mean pulmonary arterial pressure, wave reflection index, and flow heterogeneity. A multiscale fluid dynamics model successfully simulated severe chronic thromboembolic pulmonary hypertension hemodynamics and identified optimal target lesions for balloon pulmonary angioplasty.
