Key result
Transapical transcatheter aortic valve implantation in high-risk patients with severe aortic stenosis resulted in a 12-month all-cause mortality rate of 24.6%.
Why the study?
Transapical access represents an alternative route for TAVI in severe aortic stenosis patients unsuitable for transfemoral access, prompting assessment of early- and mid-term clinical outcomes.
Does transapical transcatheter aortic valve implantation provide acceptable clinical outcomes and mortality rates in high-risk patients with severe aortic stenosis?

Does transapical transcatheter aortic valve implantation provide acceptable clinical outcomes and mortality rates in high-risk patients with severe aortic stenosis?
Transapical TAVI in high-risk patients with severe aortic stenosis provides good hemodynamic results and acceptable 12-month mortality, though bleeding complications remain a concern.

Supports transapical TAVI feasibility in high-risk aortic stenosis; leaves open need for randomized comparisons to guide access selection.
INTRODUCTION: Transapical access (TA) transcatheter aortic valve implantation (TAVI) (TA-TAVI) represents one of the possible routes in patients with severe aortic stenosis (AS) who are not suitable for transfemoral access. AIM: To assess early- and mid-term clinical outcomes after TA-TAVI. MATERIAL AND METHODS: Patients with severe symptomatic AS undergoing TA-TAVI from November 2008 to December 2019 were enrolled. Clinical and procedural characteristics as well as clinical outcomes including all-cause mortality during 12-month follow-up were assessed. CONCLUSIONS: Transapical TAVI in high-risk patients provides good hemodynamic results with acceptable outcomes.

Wiewiórka et al. (2021) conducted an observational in severe symptomatic aortic valve stenosis (n=61). Transapical transcatheter aortic valve implantation (TA-TAVI) was evaluated on 12-month all-cause mortality. Transapical transcatheter aortic valve implantation in high-risk patients with severe aortic stenosis resulted in a 12-month all-cause mortality rate of 24.6%.
