This case report describes a functional double potential recorded during reentrant atrial tachycardia and entrainment pacing in a patient with WPW syndrome, highlighting specific electrophysiological characteristics.

A型顕性WPW症候群に合併したリエントリー性心房頻拍 (AT) で, 頻拍中および頻拍時に行った右房ペーシング中にdouble potential (DP) が記録された症例を経験した, 症例は頻拍発作を主訴とした42歳女性で, 電気生理学的検査により洞調律時とほぼ同じQRS波形の頻拍はAT (CL 490mseG) によるものと診断した.洞調律時の心房マッピングでは, 高位右房側壁 (HRAlat) で心房波の持続時間の延長を認めた.ATはHRAlatからの早期刺激法で誘発, 停止が可能であり, 早期刺激間隔とAT誘発時のエコー間隔との関係は逆相関であった.AT中の心房マツピングでは, 高中位右房側壁の興奮が最早期で, 中位右房側壁にDPを認めた.DPはAT時にHFAlatから行ったペーシング中 (entrainment, CL 470, 450msec) にも記録されたが, 洞調律時および洞調律時の心房ペーシング中には記録されず, 機能性のDPと考えられた.
