HSAT offers an accessible alternative to PSG in heart failure but CSA accuracy requires validation.
Why the study?
In-lab polysomnography is limited by cost and accessibility, while home sleep apnoea testing offers an accessible alternative whose accuracy in heart failure populations remains under evaluation.
Does home sleep apnoea testing accurately diagnose sleep-disordered breathing in patients with heart failure?
Does home sleep apnoea testing accurately diagnose sleep-disordered breathing in patients with heart failure?
Home sleep apnoea testing improves access to sleep-disordered breathing diagnosis in heart failure patients, but further refinement is needed to accurately detect central sleep apnoea and guide therapy in this population.
May improve SDB diagnostic access in HF; leaves open HSAT accuracy for central events pending validation.
Sleep-disordered breathing (SDB), including obstructive and central sleep apnoea, is highly prevalent in heart failure (HF) and contributes to adverse outcomes. In-lab polysomnography is the diagnostic gold standard, but is limited by cost and accessibility. Home sleep apnoea testing (HSAT) offers an accessible alternative, but its accuracy in HF populations remains under evaluation. This review explores HSAT technologies, including peripheral arterial tonometry and respiratory inductance plethysmography, and their ability to detect SDB subtypes. Differentiating obstructive sleep apnoea from central sleep apnoea in HF is complicated by overlapping physiology, comorbidities, and fluid status. With further refinement, HSAT may improve access to timely diagnosis and management of SDB in HF, potentially enhancing outcomes in this high-risk population.

Steinberg et al. (2025) conducted a review in Heart failure and sleep-disordered breathing. Home sleep apnoea testing vs. In-lab polysomnography was evaluated. Home sleep apnoea testing offers an accessible alternative to in-lab polysomnography for diagnosing sleep-disordered breathing in heart failure, though its accuracy for central sleep apnoea requires further validation.
