心不全の本態は，左室拡張期圧の上昇であるが，その圧をカテーテルによって直接計測できなくとも，現場で間接的に左室拡張末期圧を反映する指標はいくつか存在する．身体所見，画像検査所見（主に胸部x線）ならびにバイオマーカー等が一般的なものであるが，本稿では，これらを現場で活用していくためのポイントを挙げつつ，また，「令和時代の心不全診療」という特集テーマを意識し，最新の知見等も紹介していく．
