Hybrid ablation and AtriClip exclusion successfully eliminates refractory atrial tachycardia from a giant LAA.

Thoracoscopic surgical approach is a viable alternative for focal atrial tachycardia originating from the left atrial appendage after failed endocavitary ablation.
May support hybrid 3D-guided AtriClip exclusion for refractory giant LAA AT after failed ablation; leaves open prospective validation.
Key Teaching Points•The management of symptomatic atrial tachycardia resistant to drug therapy must be done by endocavitary ablation.•Computed tomography scan of the left atrium should be performed frequently. It can highlight anatomical particularity that will lead to an adaptation of the ablation procedure.•Success rate of ablation of an atrial tachycardia originating from the left atrial appendage is close to 70% according to the series. The thoracoscopic surgical approach appears to be a good alternative after failure of endocavitary ablation. •The management of symptomatic atrial tachycardia resistant to drug therapy must be done by endocavitary ablation.•Computed tomography scan of the left atrium should be performed frequently. Combes et al. (2017) conducted a case report in Radiofrequency-resistant atrial tachycardia originating from a giant left atrial appendage (n=1). Hybrid electrophysiological and surgical approach (LAA exclusion with AtriClip) guided by 3D modeling was evaluated on Restoration and maintenance of sinus rhythm. A hybrid electrophysiological and surgical approach utilizing 3D modeling and AtriClip exclusion successfully treated a patient with radiofrequency-resistant atrial tachycardia originating from a giant left atrial appendage, with no recurrence at 6 months.
