Specific ECG abnormalities help identify underlying genotypes and predict arrhythmic risk in dilated cardiomyopathy.
A careful 12-lead ECG analysis remains a highly valuable, accessible tool for genotype-phenotype correlation and arrhythmic risk stratification in patients with dilated cardiomyopathy.
This article summarizes the main electrocardiogram (ECG) findings in dilated cardiomyopathy (DCM) patients. Recent reports are described in the great 'pot' of DCM peculiar ECG patterns that are typical of specific forms of DCM. Patients with late gadolinium enhancement on CMR, who are at greatest arrhythmic risk, have also distinctive ECG features. Future studies in large DCM populations should evaluate the diagnostic and prognostic value of the ECG.

Crescenzi et al. (2023) conducted a review in Non-ischaemic dilated cardiomyopathy. Electrocardiogram (ECG) features was evaluated. Specific ECG patterns, such as conduction delays, low voltages, and repolarization abnormalities, can help identify underlying genotypes and predict arrhythmic risk in dilated cardiomyopathy.
