Overview

Supports steroid-eluting bipolar leads to extend pediatric epicardial pacing durability; leaves open need for randomized confirmation of reintervention reduction.

Structured PICO

What are the long-term outcomes and factors modifying the longevity of permanent epicardial pacing systems in children?

P
Population
119 consecutive patients from one country undergoing permanent epicardial pacemaker implantation at <18 years of age (median 1.8 years, IQR 0.3-6.4 years) between 1977-2009.
I
Intervention
Permanent epicardial pacing (including use of steroid-eluting leads, bipolar Medtronic 4968 leads, and AutoCapture feature)
C
Comparator
Older pacing technologies (unipolar 4965 leads, non-steroid eluting leads, without AutoCapture) and earlier implantation eras
O
Outcome
Probability of absence of any pacing system dysfunction and continued epicardial pacing at 5 and 10 years after implantation

In children requiring permanent epicardial pacing, the use of bipolar steroid-eluting leads and beat-to-beat capture tracking significantly increases pacing system longevity and reduces the need for surgical reinterventions.
