Key result
Cox-Maze IV during mitral and aortic valve replacement restores sinus rhythm in ~81% without requiring pacemakers.
Why the study?
The study was conducted to evaluate the immediate outcomes of mitral and aortic valve replacement performed simultaneously with the Cox-Maze IV procedure.
Does a simultaneous Cox-Maze IV procedure during mitral and aortic valve replacement effectively restore sinus rhythm?

Does a simultaneous Cox-Maze IV procedure during mitral and aortic valve replacement effectively restore sinus rhythm?
Simultaneous Cox-Maze IV procedure during double valve replacement is safe and effectively restores sinus rhythm at discharge in the majority of patients.
Supports safety of concomitant Cox-Maze IV in double valve replacement; leaves open durability and generalizability pending prospective trials.
Objective. To evaluate the immediate outcomes of mitral and aortic valve replacement with simultaneous Cox-Maze IV procedure. Material and methods. A retrospective study included 23 patients who underwent mitral and aortic valve replacement with simultaneous Cox-Maze IV procedure between 2017 and 2023. Results. Sinus rhythm at discharge was noted in 17 (81%) patients, atrial fibrillation — in 4 (19%) patients. Postoperative hospital-stay was 9 (7; 10) days. Among patients with paroxysmal AF, sinus rhythm at discharge was restored in 5 (83.3%) out of 6 patients, in persistent AF — 3 (100%) out of 3 patients, in longstanding persistent AF — 9 (75%) out of 12 patients. No cases of permanent pacemaker implantation were observed. Conclusion. Mitral and aortic valve replacement with simultaneous Cox-Maze IV procedure is effective and safe. This approach improves early and long-term outcomes, as well as quality of life.

Evseev et al. (2025) conducted a cohort in Mitral and aortic valve disease with atrial fibrillation (n=23). Mitral and aortic valve replacement with simultaneous Cox-Maze IV procedure was evaluated on Sinus rhythm at discharge. Mitral and aortic valve replacement with simultaneous Cox-Maze IV procedure restored sinus rhythm at discharge in 81% of patients, with no need for permanent pacemaker implantation.
