A scoping review reveals a critical lack of research on diabetes prevention and management for women, particularly those aged 65 and older, within US Federally Qualified Health Centers.
Introduction and Objective: Women present with unique risk factors related to diabetes, such as hormonal fluctuation effects on insulin and increased heart disease risk compared to men with diabetes. These physiological risk factors for women can become more pronounced in the setting of high social risk. FQHCs provide a unique lens into providing equitable primary care prevention and management of diabetes in women with high social risk. The objective of this scoping review was to identify gaps in women’s health research within United States’ Federally Qualified Health Centers, with particular attention to diabetes. Methods: This scoping review followed the JBI Manual for Evidence Synthesis (2024). Peer-reviewed publications were collected via PubMed/MEDLINE, CINAHL Plus with Full Text, and Scopus. Gray literature was collected via ProQuest Dissertations & Theses Global. Extracted information was organized using The Commonwealth Fund’s Framework for Transforming Primary Health Care for Women. Results: The total number of references included in this review was 160 articles. Only 5.63% of the included references (n=9) related to diabetes prevention and management for women, many of which examined diabetes in pregnant and postpartum women (n= 4 references). Additionally, of these articles investigating diabetes in women, 44.4% sampled women in early adulthood (18-44 years), followed by 44.4% sampling women from early to advanced adulthood (18-65+ years), and finally 11.1% sampling women from early to middle adulthood (18-64 years). None of the included references included diabetes research solely for women in advanced adulthood (65+ years). Conclusion: Researchers should invest in partnerships with FQHCs to further research on women in advanced adulthood, with particular attention to diabetes. This call for additional research is especially justified when considering that diabetes prevalence is associated with advancing age and social risk factors. Disclosure E.K. Larson: None. M.H.
