Retrospective cohort study identifies factors influencing GLP-1 receptor agonist prescriptions in type 2 diabetes, suggesting obesity as a key driver.
Introduction and Objective: The prescribing pattern of GLP1RAs among patients with newly diagnosed T2D is unknown. We aimed to identify factors that contributed to the providers’ decisions to initiate GLP1RA in this population. Methods: A retrospective cohort study was conducted using a health system EHR database (2013-2023). Patients with the first documentation of ICD-9/10 codes of T2D were included, and patients with missing baseline parameters were excluded. GLP1RA starters were compared to non-starters by standardized mean differences and Mann-Whitney U tests. The 10-year ASCVD risk scores of GLP1RA-eligible non-starters were also evaluated. Logistic regression was used to identify predictors of GLP1RA initiation. Results: Of the 1710 subjects who met the inclusion criteria, 577 (34%) started on a GLP1RA with a mean initiation time of 7±2 months. Compared to the non-starters, starters were younger (55±13.3 vs 59±14.5 years), had higher BMI (33.6±8.6 vs 28.9±6.7 kg/m²) and higher HbA1c (8.0±2.3 vs 7.4±2.1%)(all p< 0.001). Among non-starters, 259 (23%) were GLP1RA eligible, with 71.8% having intermediate or high 10-year ASCVD risk scores. In multivariable logistic regression, BMI was the strongest predictor of GLP1RA initiation (OR 1.07 per kg/m², p<0.001). Conclusion: GLP1RA initiation at T2D diagnosis was primarily driven by obesity, underscoring missed opportunities to incorporate cardiovascular risk reduction into treatment decisions.
