Medication errors caused by unsafe drug combinations, dosage overlaps, and poor adherence remain a major challenge in modern healthcare systems. Elderly patients and individuals with chronic diseases are particularly vulnerable due to polypharmacy and missed medication schedules. Existing medication reminder systems primarily focus on notification services and lack intelligent mechanisms for medication safety analysis and adherence monitoring. This paper presents MedSafe, an Intelligent Medication Safety and Adherence Monitoring System that combines drug interaction detection, dosage overlap analysis, adherence tracking, weekly behavioral analytics, and automated alert generation. The proposed system evaluates patient prescriptions to identify potentially harmful drug combinations, monitors medication intake patterns, and predicts non-compliance risks. Based on identified safety risks and adherence levels, alerts are generated for patients, caregivers, and healthcare providers. The system aims to reduce adverse drug events, improve treatment effectiveness, and minimize hospital readmissions. Experimental evaluation demonstrates the effectiveness of the proposed framework in enhancing medication safety and supporting informed healthcare decisions.
